The Primary Care Home concept is based on the ethos of groups of practices working together to deliver out of hospital care in the patient’s community by working in close partnership with secondary care, social care and the voluntary sector. GPW-Fed was selected by the National Association of Primary Care as a second wave exemplar site. We have organised our practice into two hubs to fit a geographical pattern to work alongside the Healthy Wirral localities approach.
On the 3rd of October GPW-Fed had the Primary Care Home launch which proved a great success. The feedback was very positive, (one patient representative commented how delighted they were that at last there is a light at the end of the tunnel) attendance was high and it included a strong presence from patient’s groups and representatives from Wirral CCG, Wirral Community Trust, Wirral University Teaching Hospital’s, Wirral Borough Council and Mental Health Trust.
